Everything You Need to Know About Nosebleeds or Epistaxis

Nosebleed, medically termed "epistaxis," is a very common entity among people that is often seen as a minor inconvenience. However, these spontaneous bleedings, originating from blood vessels in the nasal passage, can sometimes signal major health concerns or may arise due to specific triggers.
What Are the Common Causes of Epistaxis?
Nosebleeds can occur for a variety of reasons. The most common causes of epistaxis include:

High Blood Pressure (Hypertension): A consistent high blood pressure might overwork the fragile vessels in the nose, predisposing them to bleed from the nose due to rupture of the blood vessels.
Environmental Factors: Dry or low-humidity surroundings can lead to brittle nasal membranes, amplifying the risk of bleeding.
Infections: Sinus infections or bacterial infections can get into the mucosa and cause nasal tissue inflammation, triggering nosebleeds.
Accidental Injuries: Any forceful impact or simple actions like nose picking can lead to bleeding.
Specific Medications: Drugs affecting blood consistency, such as anticoagulants or nasal sprays, can heighten the risk.
How to Stop Nosebleed at Home?
Here are some steps to take at home to stop a nosebleed:

Pinch the Nose: Gently pinch the nostrils together with your thumb and forefinger and tilt/bend your head down. Hold for about one minute to apply pressure to the bleeding vessels and stop the bleeding.

Lean Forward: Avoid tilting the head backward. Instead, lean forward slightly to prevent blood from flowing down the throat.

Cold Compress: Apply a cold compress or ice pack to the bridge of the nose to constrict blood vessels and reduce bleeding.

Stay Calm: Maintain a calm demeanour, as anxiety and stress can worsen the situation by increasing blood pressure.
When to Seek Medical Attention?
If the nosebleed does not stop within 15-20 minutes, or if it is severe, you should seek immediate medical attention.
Here are some situations when you should rush to the hospital:

Regular Episodes: If nosebleeds become recurrent (or occur often), it is time to consult a doctor.
Mysterious Origins: A medical check-up becomes crucial when there is no clear cause behind the nosebleed.
Persistent Bleeding: A nosebleed that doesn't stop might indicate a deeper medical emergency.
Existing Health Concerns: People with conditions that affect blood clotting or those on specific medications should always consult a doctor when getting nosebleeds.
Medical Management of Epistaxis
When you visit emergency care for nosebleeds, they will check your vitals, and if everything is under normal range, they will refer you to the ENT department. The ENT doctor will then conduct a thorough examination to find the cause of epistaxis and begin appropriate treatment. Medical management of epistaxis can range from simple epistaxis first aid measures to more invasive procedures, depending on the severity and cause of the bleeding.
Epistaxis First Aid and Basic Medical Care
1. The doctor will put direct pressure by pinching the nostrils together to stop the bleeding as it remains the first-line treatment for anterior bleeds.

 2. Sometimes, commercially available nasal tampons or gauze packs will be used in the nasal cavity to compress the bleeding site. Antibiotics are necessary to prevent any infections if they are the reason behind these sudden nosebleeds.

3. Topical vasoconstrictor agents like oxymetazoline or phenylephrine can also be applied topically to constrict the blood vessels and stop bleeding.
Advanced Medical Interventions
If first aid care is unable to stop the nosebleed, then medical interventions are suggested to treat the issue.

1. Cauterization: Electric cauterization can be effective for identifiable bleeders, especially in the anterior septum.

 2. Anterior Nasal Packing: Anterior nasal packing involves placing a more extensive pack or balloon device into the anterior nasal cavity.

Follow-Up
After initial treatment, you may be asked to return for a follow-up appointment within 24 hours to ensure the bleeding has stopped and monitor your progress.
How to Prevent Epistaxis?
To minimise the risk of nosebleeds, consider the following preventive measures:

Use a humidifier to maintain adequate indoor humidity, especially during dry and cold seasons.

Regularly use a saline nasal spray to moisten the nasal passages and prevent dryness.

Avoid picking your nose, which can damage delicate nasal tissues.

Wear appropriate protective gear during activities that pose a risk of facial injuries.

Manage Hypertension. If you have high blood pressure, work with your healthcare provider to control it effectively.
